Setting Up A Custom PDF
1. Select the Stack icon near the Form Name.
2. Turn on the flag labeled Use Custom PDF.
3. Select the Files and Mapping button.
4. Select Upload a PDF, then select your fillable PDF file.
5. Select the Configure Mapping button. The mapping modal will appear.
Radio Buttons
Fillable PDF Radio Buttons need to be setup with a Radio Grouping with all options added underneath. Radio buttons need to be mapped to the Option Answer Type in Omnidek.
Checkbox
Fillable PDF Checkboxes need to be mapped to the Yes/No Answer Type in the Form Designer.
1. Add a checkbox to the PDF
2. Go to Properties > Options
3. Set Check Box style to Check
4. Set Export Value to 1
Signatures
Signatures can be setup using a Text Field on the PDF mapped to the Signature Answer Type in Omnidek. The mapping will pull the user's name as text on the PDF. We suggest adding agreement text stating that the name on the PDF is acting as a signature.
